[PATCH] connection adapters should be reconfigured on reconnect!

(In [3165]) MySQL, PostgreSQL: reconnect! also reconfigures the connection. Otherwise, the connection 'loses' its settings if it times out and is reconnected. Closes #2978.

Thanks for fixing the PostgreSQL adapter also! It was our mistake. Connection adapters need to make their own connections rather than accepting them in initialize. The current way is messy. We will revise it.
